This is constipation management in a nutshell. Of course, if you have pelvic floor involvement with your constipation you could be doing all of these strategies seamlessly and still need more assistance. If constipation is still a challenge, make sure to schedule pelvic PT appointments to improve your constipation.

This is not only important to be able to eliminate more efficiently, but also can help to prevent further issues down the road. Concerns such as hemorrhoids, a**l fissures, pelvic organ prolapses, pelvic pain in the front or the back, or referral pain to various areas.

These are just the basics of how to reduce a flare. If you struggle with pelvic pain, working with a pelvic PT and other medical professionals is the best way to navigate pain. Treatments will be very specific for who you are, what triggers flares, and what your needs are based on your current and past history.

Patients never regret starting early. Most tend to regret not starting soon enough.

This just recently happened to a patient. She no longer had pain with in*******se and constipation and elimination had been great. We had been focusing more on some other symptoms around exercises with most recent treatments. Then the pain with in*******se and constipation came back.

We looked at all things she normally does to any new or deviation from her normal routine. Biggest changes, more travel and working onsite for work instead of home. During her treatment we went ahead and treated for the return of pain symptoms. We also addressed some of the daily things that had changed to better support pain and digestion.

Her change in sleep, increased pace in morning routine, and less time for p**ping time at home were some major differences for her change in work routine. Sometimes the small subtleties in life are what can cause triggers in symptoms.

We do not freak out.

Sometimes flares happen and the biggest goal if this does happen is to better understand the driver(s) of the symptoms and make changes to improve the current symptoms and make future events better. It can be super individualized when approaching managing flares.

The most commonly recommended strategies for constipation are drinking water and eating fibrous foods. You need both to have a good consistency in p**p, otherwise your p**p could still be firm and constipated.

To focus even more on better p**ps adding in daily movements can improve how the p**p moves through your digestive system.

Improving quality of sleep and reducing stressors helps to regulate hormones, normal body system, and stimulates the parasympathetic nervous system aka rest and digest.

Add in treating the pelvic floor for making sure the pelvic floor muscles have the ability to lengthen to accommodate for the p**p to pass through the ca**l and add coordination so elimination can happen without straining and reducing risk of pelvic floor injury.

Top that off with any additional pelvic floor PT interventions from an external standpoint and there is a very comprehensive management for constipation.

This conversation just came up last week for a patient I have been treating for her third postpartum journey. From home schooling her kids, to all the house things, and taking care of a family all of the time her time is stretched super thin.

What did we do? We broke down her regular weekly or daily tasks and made her home exercise program structured around what she already has to do. If time becomes available she has specific exercises to do too. Keeping it functional in her time scarcity is crucial as it is for so many of our patients.

It is possible to get better even if you are stretched for time. Your treatments should be intentional and functional to make strides towards your goals.

Do you have to do kegels in isolation to get them stronger?
No, but you can if you want. My friend, don’t forget about function when you think of the pelvic floor.

Literally any exercise or movement can be done to strengthen the pelvic floor muscles. This also means that your pelvic floor muscles should be coordinating the proper squeeze and relax patterns for said movement or exercise.

If you have symptoms of leakage, prolapse or pelvic pressure, or pain then diving into if the muscles are engaging correctly will be essential in improving symptoms. If you have symptoms with a particular activity or movement you better bet that your exercise routine should be specific to train around those activities to prevent symptoms.

Where do you even start? Work with a pelvic PT so you can take all of the guessing work out of figuring out what is best for you. Then as you progressively get better your pelvic PT will change your home exercise program and in clinic program to work towards your goals.
