Patient Care Manager

Born and raised in New Mexico, Chasie overcame many obstacles to pursue her dream of becoming a Registered Nurse. For the past 15 years, she has dedicated her career to caring for others with compassion, strength, and heart.

Over the last 9 years, Chasie has found her passion in hospice care, providing compassionate end-of-life support to patients and families during life’s most meaningful moments.

Since joining the Bosque Trails Hospice team in 2023, Chasie has continued to make a lasting impact through her kindness, dedication, and commitment to exceptional care. 💙

Managing pain while on hospice ❤️

Hospice care focuses on improving day to day quality of life instead of trying to cure illness. Pain management is often a big part of this.

Your Bosque Trails Hospice team will learn about you and your goals and values and explain different options to treat your pain. A plan will then be made together with you and your loved ones.

Treating pain with opioids and other non-opioid medications can improve quality of life and comfort in more areas than physical pain. It can help with mood, sleep abd anxiety and allow you to participate more meaningfully with your loved ones.

Pain management will not hasten death. Our hospice and palliative medicine board certified doctors will carefully prescribe and adjust opioids when needed for your pain. Opioids allow for comfort and increased quality of life for many patients.

Meet Joanna Brightwater, AGPCNP-BC
Nurse Practitioner

Joanna Brightwater, CNP moved to New Mexico initially in 2002 after serving in the United States Coast Guard as Corpsman. She then moved away temporarily while transferring with family who remained on active duty. During her time away, she decided to continue her healthcare career and pursued her bachelor's in nursing and graduated from the University of South Alabama. She then continued her education through the University of Maryland, Baltimore and obtained a master's in science. She is certified as an Adult-Gerontology Primary Care Nurse Practitioner. She returned to New Mexico in 2013 and considers NM to be home. Joanna has practiced in Hospice care since 2021 and is appreciative to be invited to share time with patients and their families in the sacred space of end-of-life care.

This journey requires compassion, understanding, and specialized care which our board certified hospice and palliative medicine physicians are trained to give.

Our team is dedicated to providing comfort, dignity, and personalized support for patients facing advanced stages of Parkinson’s. We are here to ensure that no one faces this journey alone.

This month, we raise awareness, share knowledge, and stand in solidarity with everyone affected by Parkinson’s. Together, we can make a difference through empathy, education, and superior care.
