Dr. Kandil, does cupping work to heal sore muscles? What’s the best remedy for sore shoulders from playing?

Cupping has become more popular over the past decade, thanks in part to Michael Phelps in the 2016 Summer Olympics. Cupping dates back to 1550 BC as described in the Egyptian Ebers Papyrus and is used by many cultures including Chinese, Arabic, and South Asian cultures.
The negative pressure from cupping results in swelling and rupture of the blood vessels under the skin leading to the characteristic bruising. It has been suggested that cupping enhances skin circulation, loosens adhesions and lifts connective tissue, changing the biomechanical properties of skin, and altering pressure pain thresholds.
The current literature may support the use of cupping in treating pain associated with nonspecific neck, back, and shoulder pain, but the quality of evidence is low. The risks associated with cupping are low enough that it is a reasonable alternative treatment option for athletes experiencing back, neck, or shoulder pain.
The best two remedies for sore shoulders are rest and ice. Rest allows for the overexerted muscles to take a break and recover from the exercise or activity. Ice or cryotherapy will help improve pain and decrease inflammation, causing less pain and soreness during the recovery process. Reported benefits of cryotherapy include the promotion of local blood constriction to control swelling, as well as the reduction of pain and muscular spasm. I recommend using cryotherapy for 20 minutes after an exercise or sporting session.

This patient saw multiple doctors and was told there’s no good treatment option for his knee pain. I did a thorough evaluation and recommended an osteochondral allograft transfer for their large osteochondral lesion. Patient is now pain-free and back to activities he enjoys.
Knee cartilage injuries are challenging to treat. There are a variety of different techniques and procedures for various cartilage lesions. One type of cartilage lesion is called the osteochondral lesion.
“Osteo” means bone and “chondral” means cartilage. An osteochondral lesion is therefore a cartilage injury that involves the underlying bone. This is analogous to a a damaged home with a damaged foundation. While one can rebuild a house on a solid foundation, building a house on a poor foundation is a recipe for disaster.
For large , a common treatment option is called an . This involves finding a size and location matched donor graft for your patient’s cartilage lesion. This is very similar to procuring a kidney or heart transplant. Using a donor graft from a donor with a larger knee will lead to a mismatch and potential for graft failure. The match must be a .
Patients may wait weeks or sometimes months to get a new graft. Once a graft is matched and procured, we perform the procedure of removing the osteochondral lesion using a cylindrical reamer and procuring a perfect match from the donor. We then remove all marrow elements and I enhance the healing potential by using the patient’s own ( ). After making final measurements to ensure size and depth are perfect, we transplant the graft into the patient.

Finger dislocations are common sports injuries. They often occur when a finger is hyperextended and/or compressed axially. Dislocations lead to tearing of the ligaments surrounding the finger joint.
There are two main types of finger dislocations: complex vs. simple. Complex dislocations are finger dislocations with a fracture, whereas simple dislocations have no fracture. The distinction is important because having a fracture makes the finger more unstable, increasing the chance of surgical treatment.
Treatment for simple, stable finger dislocations is simply buddy taping for 3-4 weeks. For unstable finger dislocations, a special type of splint called the extension block splint is used for more stability.
