Dear Valued Patients,
It is with mixed emotions that I announce my retirement from active practice, effective November 30th, 2024. It has been a great pleasure providing for your health care needs over the years, and it is not easy for me to give it up; however increasing health issues have left me little choice.
I am currently working with Alex Vision Source in Alexandria to transfer all patient charts. This will ensure a seamless transfer of care and I am fully confident Dr. LaMar Gunnarson, Kay Gunnarson, Dr. Scott Minnerath and their staff will take excellent care of you. If you prefer to have your records transferred to another facility, please contact Amy or Jodi at our office before November 30th and they will assist you. After that date please contact Alex Vision Source at 320.219.6543 for your continuing care.
Should you have any questions, we would be happy to assist you. Please note that we plan on the office being closed permanently as of December 15th, 2024. Please keep this in mind and allow enough time for us to assist you with any records requests, medication refills or purchases. If you would like to refill a medication, order contacts, utilize a lens warranty or make any other purchases, we strongly recommend making sure the order is placed no later than November 15th, 2024. I have come to know many of my patients as friends and I am honored you trusted me with your eye care needs. The past 33 years have been a rewarding experience and I will greatly miss all of you.
