Back to school supply drive! Yes that time is already here! Choice Providers is hosting our first Back to School supply drive for children in our community! Backpacks, pencils, pens, paper, markers!!! All of the things!! Donations can be dropped off at our office 2006 Gus Kaplan Dr. Ste. A in Alexandria. Everyone who donates will have their name entered into a drawing to win a $50 gift certificate.

❕Healthy Louisiana Open Enrollment began on October 15 and ends at 6 p.m. on December 2❕

During Open Enrollment, Medicaid members can change their health and dental plan. Changes will go into effect on January 1, 2025.

✉️Members were mailed an open enrollment packet with instructions and other helpful information. Members may receive this information by email or text message if they opted in to that service.

📱Get tips for choosing the best plan for you and your family and learn about extra benefits each plan offers at www.myplan.healthy.la.gov or on the free Healthy Louisiana Mobile App.

❓If you have questions or did not receive your Open Enrollment information, call 1-855-229-6848 (TTY: 1-855-526-3346). The Healthy Louisiana hotline is open Monday – Friday, 8 a.m. to 5 p.m.
