Primary functions of the Hospice Intake Coordinator / LVN position are:

This position serves as the admin support for the Ardent Hospice Intake Center. This position ensures the effective communication between referring physicians, board and care facilities, skilled nursing facilities, patients, families, hospice staff, marketing staff, other referral sources and the Ardent Hospice organization. The Intake Coordinator assists in managing the intake phone line and documenting hospice referrals (electronic). The Intake Coordinator assists with scheduling assessments, consistently communicates with all parties regarding new referrals and supports statistics and reporting functions for the Leadership Team.
