Grane Hospice provides a higher level of hospice care through a low patient-to-nurse ratio, allowing for more care time whenever needed.

Our focus is to provide comfort and dignity when faced with a terminal illness. Grane Hospice provides a team-oriented approach to care that includes medical, social, emotional and spiritual support during a difficult time. Currently serving the following counties:
Bedford, Blair, Cambria, Centre, Clearfield, Fulton, Huntingdon, Indiana, Mifflin, and Somerset

We know pharmacists as the professionals we go to to get our medication, but they also are key to helping patients like ours manage complex medication regimens by:

• Reviewing prescriptions to make sure drugs are safe together and dosed correctly
• Helping manage side effects and adjust timing so medicine fits into daily routines
• Preparing special, labeled packaging for ease of use
• Consulting with other medical professionals to adjust patient plans as necessary
