Dear Valued Patients,

It has been brought to our attention that there is false information being spread about our recent closure. I would like to let everyone know that our closure has absolutely nothing to do with being under investigation. This is happening solely due to an unfair financial recoupment issue that we cannot overcome. This is also happening to thousands of doctors across the nation. Please read the attached photo for more information and refrain from spreading falsified information regarding our office. I am grateful to have been able to serve our community for the last 16 years and appreciate each and every one of you.

Dear Valued Patients,

I am writing to convey with deep regret and a heavy heart that Southwest Georgia Center of Medicine will no longer be able to provide medical services to our cherished patients. After 15 years of dedicated practice, we find ourselves compelled to cease operations due to what we perceive as uncompassionate and unjust actions against our organization by CMS (Medicare).

Our last day of providing medical services will be on December 6th, 2023. Should you wish to transfer your medical records to another healthcare provider, kindly complete our Transfer of Records Authorization Form. Subsequently, once the office is closed, your medical records will be securely stored at 915 Elmo Street, Americus, GA 31709, until the property is sold. Access to your records will be available at Columbus Physical Medicine Center (118 Enterprise Court, Suite B, Columbus, GA 31904, phone # 706-330-1389). Requests will be processed in accordance with the time limitations stipulated by state and federal law.

We extend our heartfelt gratitude for entrusting our team with your healthcare during our time at Southwest Georgia Center of Medicine. It has been an honor and privilege to care for you. We sincerely wish you continued health and happiness in all your future endeavors.
