We are seeking dedicated and compassionate professionals to join our team as CBRS and Case Management providers. This role focuses on supporting individuals in achieving greater independence, stability, and quality of life through personalized, strengths-based services. This position will serve clients in the greater Idaho Falls area, working with both children and adults. The CBRS/CM will be responsible for linking clients to services in the community, coordinating care, and providing vital support.

โขCase Management: Coordinate services and assist clients in accessing necessary resources, including helping arrange transportation.
โขClient Support: Collaborate with families, doctors, and therapists to ensure effective communication and care.
โขSkill Building: Provide instruction in various skills across different settings, including office, home, and community environments.
โขDocumentation: Complete weekly client notes, 90-day reviews, and annual plans.
โขCollaboration: Work effectively with other clinicians and staff, ensuring a seamless client experience.

โขBachelorโs degree or higher in a humanities-related field required
---Relevant backgrounds may include (but are not limited to): education, social work, psychology, sociology, physical therapy, counseling, or related disciplines.
โขStrong interpersonal, organizational, and communication skills
โขAbility to prioritize responsibilities and coordinate within a team-oriented environment.
โขKnowledge of, or willingness to learn, HIPAA regulations and ethical guidelines.
โขPassion for helping individuals and families thrive
