Most know that dopamine agonists are recommended AGAINST in the treatment of restless legs syndrome (RLS) due to augmentation, an insidious worsening of the condition over time. However, impulse control disorders are an equally devastating effect of the drug class. Noel Titheradge in his second investigative piece on these drugs for the BBC details the disturbing sexual impulse problems that are common but often fly under doctors' radars.

For those who have been "stuck" (i.e. chemically dependent) on benzodiazepines like clonazepam, lorazepam, and alprazolam for sleep conditions like insomnia and restless legs syndrome (RLS), the American Society of Addiction Medicine - ASAM has released their benzodiazepine tapering guidelines, which was also endorsed by the American Academy of Sleep Medicine (AASM).

The 2025 American Academy of Sleep Medicine Guidelines for the Treatment of Restless Legs Syndrome (RLS) recommend against the use of clonazepam in the treatment of RLS, which is still quite common. This detailed The Wall Street Journal piece supports why the downsides may outweigh the little benefit demonstrated by theses benzodiazepine drugs for RLS. You may ask, then, why are opioids conditionally recommended in the same Guidelines? Unlike benzos that have little benefit for RLS other than sedating people with RLS, opioids at very low doses are the most effective treatment for RLS, which tips the balance of benefits versus harms in the favor of opioids being more beneficial than harmful. In any case, every drug has its risks, which is why there should be a greater focus on iron in the treatment of RLS before medications are even considered.

Amid rising concerns about benzodiazepines, some patients who try to quit are suffering extreme anxiety, memory loss and intense physical pain.
