Bariatric surgery is being recognized as the most effective method of long term treatment for morbid obesity.
Procedures such as Gastric Bypass, Sleeve Gastrectomy or Duodenal Switch have helped improve or resolve co morbid conditions such as Type 2 Diabetes, hypercholesterolemia, hypertension and sleep apnea.
In approximately up to 25-30% of patients may experience acid reflux after weight loss surgery.
This webinar is intended to review both conservative and surgical management of reflux following today’s most common bariatric techniques.
Teaching Points:
- Why do we have reflux (GERD) before or after surgery
- Surgical correction of post operative GERD
- Conservative Management
- GERD as a source of weight regain

Bariatric surgery has been recognized by several authors as a long-lasting and durable management for the treatment of morbid obesity. The main objective of these operations is to resolve or improve co morbid conditions such as Type II Diabetes, High blood pressure, high cholesterol and Sleep Apnea.

Despite of the excellent results achieved during the first two years (60-70% of excess weight loss) the incidence or weight regain or recidivism ranges between 20%- 35% and it is usually is the result of several factors including: Aging, Genetics, sedentary lifestyle and setbacks with the original surgery.

Weight Regain after Gastric Bypass:

According to studies weight regain can be seen in 30% to 35% of patients following a bariatric surgery.

The causes are multifactorial and may include:

1- Adaptation of your body to the surgery
2- Aging
3- Heartburn as a result of a "Hiatal Hernia"
4- Big pouches and sleeves

Following an initial consultation, certain tests may be needed to understand the anatomy and therefore suggest a potential surgical correction if granted.

There are several procedures that can be offered to optimize a former procedure (Sleeve or Gastric bypass).
Today I am going to touch on a technique known as “Distalization" used to revise a Gastric Bypass that did not yield the expected results.

This procedure intents to enlarge the length of functional bowel (biliary limb) to create less absorption surface and to shorten the functional bowel, hence loosing calories. The procedure usually takes 20-25 min and it is done under anesthesia.

This picture shows how to cope with certain nutrients based on their caloric density (high calorie vs Low calorie). While foods with a “GREEN LIGHT” are part of a low caloric group and should be eaten freely, be careful with those on the “RED LIGHT” as they will interfere with your weight loss goals.
