Jeanine “Gnee” Hoffmann-Neumeyer 💙

Hi everyone! I’m Gnee, one of the Financial Navigators here at Partnership Community Health Center. My role is to help patients and families navigate insurance, coverage options, community resources, and financial assistance programs so healthcare feels a little less stressful and a lot more manageable.

A little about me:

🦷 I have a background in medical, dental, and insurance billing
👩‍👦 I’m a mom- Oliver is turning 3 in June!
✈️ I love to travel and experience new cultures, foods and adventures!
📚 My goal for 2026 is 50 books read, I’m already 1/2 way there!
🤝 I love helping people feel heard, supported, and informed
📋 My favorite part of this job is helping patients find solutions they didn’t know were available
☕ My desk usually has picture frames, candy to share, paperwork, and about 47 sticky notes at any given time
🧡 I believe healthcare should feel compassionate — not confusing

As a Financial Navigator, I can help with things like:

Today, all Partnership Community Health Center locations are closed in observance of Memorial Day.

Today, we pause to honor and remember the brave men and women who made the ultimate sacrifice while serving our country. Their courage and dedication will never be forgotten.

As we spend time with family and loved ones today, we remember the true meaning of Memorial Day and those who gave everything for our freedom. ❤️🤍💙

Our Sliding Fee Program helps make medical, dental, and behavioral health services more affordable based on household size and income. No judgment. No shame. Just care for our community.

Healthcare shouldn’t be a luxury. It should be accessible, compassionate, and centered around helping people feel their best.

If you’ve been putting off care because of cost, we encourage you to reach out and see if you qualify. You may be surprised by the options available to you. ✨

To determine eligibility, please provide proof of income dated within 30 days of your appointment. Accepted documents may include:

We’re proud to partner with The Monthlies Project to help provide free menstrual hygiene products to our community. 💗

Access to period products is a basic need — yet for many individuals and families, these essentials can be difficult to afford or access. Through this partnership, patients and community members can take what they need, no questions asked.

Small acts of support can make a big impact on someone’s dignity, comfort, health, and confidence.

The Monthlies Project is a nonprofit/community-driven initiative focused on fighting “period poverty” by providing free menstrual hygiene products to people who may not have reliable access to them. Their goal is to make pads, tampons, liners, and other essentials available in schools, clinics, shelters, community centers, and other public spaces — because period products are necessities, not luxuries.

Thank you to The Monthlies Project and everyone who helps make resources like this available in our community. Together, we’re working to remove barriers and support healthier lives for all.

Partnership Community Health Center is excited to team up with New Hmong, Zion Lutheran Church- ELCA and Multicultural Coalition, Inc. for a project that’s about so much more than gardening. Together, we’re helping create a community space where culture, connection, fresh food, and neighbors can grow side by side. 💚

This growing season, volunteers will help build 8–9 garden plots that will provide fresh produce for local food pantries and families in our community. Whether you’re a gardening pro, handy with outdoor projects, or just looking for a meaningful way to give back, we’d love to have you there!
