K an D Pharmacy patients and friends: the world of pharmacy has been turned upside. CMS (Center for Medicare and Medicaid Services) changed the rules regarding DIR(direct, indirect remuneration) fees, fees that PBM’s (pharmacy benefit managers)can charge pharmacies beginning Jan 1, 2024. The DIR fees to pharmacies and patients increased by 107,000% from 2010-2020, and the last three years hasn’t been any better.
