We’re thrilled to announce that S & S Personal Care Home will begin accepting Medicaid Residents starting October 16, 2025! 🎉

This exciting step allows us to expand our mission of providing compassionate, high-quality care to more seniors and adults throughout Georgia. Our dedicated team is here to ensure every resident receives the love, dignity, and comfort they deserve in a warm, home-like environment. 🏡💖

📝 Resident Admission Steps

Here’s how to get started with the Georgia Medicaid admission process:

1️⃣ Initial Inquiry & Screening
Call us at (229) 537-0216 or email sscare1@yahoo.com to discuss your care needs and eligibility.

2️⃣ Schedule a Tour
Come visit us, meet our amazing care team, and see what makes S & S feel like home! 🌼

3️⃣ Complete a Medicaid Application
Apply online through Georgia Gateway or visit your local DFCS office for assistance.

4️⃣ Submit Required Documents
✅ State-issued ID or Social Security Card
✅ Recent medical records & physician’s report
✅ Current TB skin test result or chest X-ray (required by GA DCH)
✅ Medicaid approval letter or proof of application
✅ Medication list & current care needs assessment

⚠️ Important: Step 4 is the most important step! Without these documents, we cannot move forward with your admission process.

5️⃣ Assessment & Care Plan
Our care team will create a personalized plan tailored to each resident’s needs.

6️⃣ Admission & Move-In
Once everything is complete, we’ll help you move in and settle comfortably into your new home. 🏠✨
