Park Avenue Nursing & Rehabilitation Center is dedicated to providing quality outcome focused therapy services for all its residents. The short-term rehabilitation program is available seven (7) days per week and offers comprehensive rehab services. Long-term care residents receive therapy services to maintain their highest level of functioning. Utilizing input from a

Outcome Analysis Reports
All of our patients receive a comprehensive evaluation detailing their clinical and functional status at time of admission and discharge. A weekly goal tracker is used to inform caregivers, the patient and family members of progress made toward short and long-term goals. Ask about our most recent results as compared to regional statistics. During December 1, 2007 through November 30, 2008, 76% of the patients participating in the Short Term Rehabilitation Program either went home or reentered the community post-discharge from services.
