Meet Terlisa Phillips, CNA
Hospice of Clay County

“I have been a CNA for 29 years and have worked in nursing homes for the majority of my career. I’ve always loved making my patients’ day better if possible and letting them know that I am here for them. I joined Clay County Hospice — to continue doing what I love, just a little closer to home.”

Meet David Holcombe, Chaplain
Hospice of Clay County

David has served as the Hospice Chaplain for more than a decade, offering comfort, prayer, and support to patients and families during some of life’s most sacred moments.

When asked why he loves working in hospice, he simply replied:

“Because God called me to do it.”

Meet Sady Moore, RN
Hospice of Clay County

“I have been a nurse for 8 years. I’ve worked in Med-Surg, Detox, CCU, and briefly in a clinic setting. I’m new to hospice, just recently starting on November 3rd of this year. Even though I’m just beginning this journey, I already feel at home, and I’ve been welcomed by the hospice team with open arms. I’m excited for this new adventure!”

“I love working locally. I’m a Clay County native, born and raised. Many people in this community have watched me grow — both as a person and as a nurse. It brings me a sense of pride to give back to my community and care for those who are in need.”

Meet Erica Wood, Social Worker
Hospice of Clay County

“I have been a social worker for 27 years. I fell in love with hospice care while in college during my volunteering and internship hours. After graduating, I worked in various medical social work positions, but I was able to come back home to Hospice of Clay County in 2008.

I spent the next 10 years in hospice before trying long-term care for a while. Thirteen months ago, I was given the opportunity to return ‘home’ to Hospice of Clay County, and I am so thankful that door opened for me again!!

I love Hospice of Clay County because it’s like we are neighbors taking care of neighbors.”
