We are pleased to announce that Lewis Family Care, under the guidance of Chad Rogers, FNP-C, a Licensed Medicinal Cannabis Practitioner, is now offering Medicinal Cannabis Certification for eligible in-house patients. This certification allows patients to legally purchase and carry medicinal cannabis within Kentucky, in compliance with state regulations.
There will be no fee involved over your regular office visit.
Who Can Qualify?
Per Kentucky law, qualifying conditions include:
• Cancer (any type or stage)
• Chronic, severe, or debilitating pain
• Epilepsy or seizure disorders
• Multiple Sclerosis (MS), including muscle spasms
• Chronic nausea or cyclic vomiting syndrome resistant to other treatments
• Post-Traumatic Stress Disorder (PTSD)
You must have written documentation of your qualifying condition.
Important Information
We are not accepting new patients solely for medicinal cannabis certification.
If you are seeking a healthcare provider, we welcome you to join our practice. Once a bona fide patient-provider relationship is established, we can evaluate your eligibility for medicinal cannabis.
