Fresh Start Health Grayson is looking for a fulltime Targeted Case Manager.

Required to have a Bachelor's Degree in a Health Services field. Prefer someone who has done TCM work in Kentucky with SUD clients.

Competitive hourly pay plus monthly bonus. Monday-Friday 8:00am-4:00pm. Paid holidays and PTO accrual. Benefits package available including 401-K. STAR Loan Repayment Plan available for post educational student loans.
