🎖️Announcing the 4 year anniversary and rebrand of the Just Ask podcast-a healthcare podcast that provides evidenced-based information by a multidisciplinary team of women’s health experts in order for individuals to make informed and optimal healthcare decisions and to learn and understand the issues that surround women throughout the lifespan.
🎖️We believe in the creation of a safe and non-judgmental environment around women and gender related health concerns. We also believe in engagement with experts in this area of health and to have access to other members of the community to discuss women’s most intimate topics and questions.
We look : To promote the understanding and advocacy of conversation surrounding women’s midlife concerns, s*xual health, metabolic and hormone wellness. We utilize multidisciplinary experts in the field to include a biopsychosocial approach to health that involves nursing, medical, pharmacy, pelvic floor physical therapy, and mental health expertise.
To debunk myths and taboos surrounding various women’s health concerns around the lifespan and other intimate topics while providing a safe and non-judgmental space for conversation.
To advocate for an evidence-based, trauma-informed approach to educating members of the community and other clinicians on women’s health topics often overlooked and underemphasized in nursing and medical education.
To improve the healthcare landscape, advocating for education, accessibility and shared decision making and empowerment.

About our Guest:
Jackie Piasta is a Vanderbilt University-trained Women's and Gender Health-related Nurse Practitioner who has been serving women for over 15 years. She currently owns Monarch Health, a holistic telehealth company focused on helping women thrive through midlife and menopause so they can feel confident and empowered as they navigate their wellness journey. Through her time in the medical field, Jackie has seen gaps in the healthcare system, particularly when it comes to women’s health. She believes that women deserve to be seen and heard when voicing their health concerns, and she is on a mission to not only hold that space but also to leave the healthcare system better than she found it. One of the biggests concerns she faced with her patients over the years was their complaints regarding their thyroid. Whether it be a concern of a possible thyroid issue contributing to their symptoms or feeling that their current care was suboptimal, Jackie has met thousands of women struggling with traditional thyroid concerns and feeling like something was missing in their care. This combined with an overall desire to improve people's overall health led Jackie to take a deeper dive into hormones and expand her knowledge through the Academy for Innovative and Preventative Medicine and their Bioidentical Hormone certification. Here, she learned evidence-based strategies for address a wide-range of hormonal concerns, thyroid included and she is here to share her knowledge with us today.

We had to repost this from our amazing colleague and former podcast guest ! All about Hormonally-Associated Provoked Vestibulodynia.

Hormonally-Associated Provoked Vestibulodynia (Ha-PVD) describes pain at the entrance of the va**na (vulvar vestibule) due to irritation and dryness of the greater and lesser vestibular glands (the glands that produce our natural lubrication and keep this skin healthy). These glands can get red and irritated when there is a lack of estrogen and testosterone, coupled with someone having "weak" testosterone receptors that do not bind hormone well.

Here are some indicators that pain with insertion can be related to hormones and vestibular glands:

Menopause- average age of menopause is 51-52. In menopause, the ovaries decrease their production of estrogen and testosterone. This can lead to whole-body symptoms (hot flashes, night sweats), but also local va**nal symptoms (dryness, pain with in*******se, UTI). This is called Genitourinary Syndrome of Menopause (GSM), which is the newer term for vulvova**nal atrophy.

Peri-menopause- describes the fluctuation of s*x hormones that occurs in the 10 years leading up to menopause. Vaginal symptoms can occur during this time as well.

Oophorectomy- removal of the ovaries (which produce a bulk of the estrogen and testosterone) has similar effects to menopause, but more sudden.

Postpartum- estrogen levels are high in pregnancy and drop with delivery. Breastfeeding suppresses o***y function and keeps estrogen levels low in the postpartum period. This can lead to va**nal dryness and pain with in*******se.

Irregular menstrual periods- lack of menstrual period may be an indicator of low estrogen and testosterone, leading to symptoms similar as described above.

Anti-androgen medications- any medications (birth control pills, spironolactone) that lower testosterone or increase S*x Hormone Binding Globulin (SHBG) effectively decrease free, or working testosterone and can lead to va**nal dryness and pain with in*******se. Those with "weak" testosterone receptors are more likely to have these side effects.
