The first lumbar laminectomy was performed by Dr. Victor Alexander Horsley, a British neurologist and surgeon, in 1887. Dr. Horsley is credited with pioneering various neurological procedures, including the laminectomy, which involves removing the lamina (part of the vertebral arch) to relieve pressure on the spinal cord or nerves in the lower back. The first lumbar laminectomy was on a patient named Frederick Smith. Frederick Smith suffered from spinal tuberculosis, also known as Pott's disease, which resulted in spinal cord compression. The successful surgery by Dr. Horsley helped relieve the pressure on Smith's spinal cord and alleviate his symptoms.

The first recorded craniotomy in modern medicine was performed by Sir William Macewen, a Scottish surgeon, in 1879. Macewen is credited with pioneering many neurosurgical techniques and advancements during his career. He conducted the first successful intracranial surgery for the removal of a brain tumor. This landmark procedure marked a significant milestone in the field of neurosurgery and paved the way for further developments in surgical techniques for brain disorders.

In 1879 he performed the first successful intracranial surgery where the site of the lesion (a left frontal meningioma) was localized solely by the preoperative focal epileptic signs (twitching of the face and arms in the opposite site of the lesion). On the basis of these signs Macewen thought that there was good evidence of an "irritation to the lower and middle portions of the ascending convolutions…in the left frontal lobe". Macewen (standing, left) became one of the world's most famous surgeons, developing a strict aseptic regime in the operating theatre.
