Our story begins in my home town of Sparta, Ga.

Hello from Heritage Oaks Structured Family Caregiving,

Here at Heritage Oaks would like to provide you more information on why we do and what we do. It was in Sparta where my grandmother Mina Flournoy Heath, who was my very first care giver lived. My grandmother Mina took care of me while my mom worked at the local elementary school. There at grandmother's house I took my first steps, got my first tooth, lost my first tooth, learned my abc's and just was over all loved and cared for. As I became older, I came to appreciate my grandmother's love, care and wanted to return that same love and care when she was in need. That very time came when grandmother needed me to care for her in her later years of life . As she got older, her mobility became limited and dementia had taken a toll. I knew it was time for me to jump in and care for my grandmother Mina in the comfort of the family home surrounded by family. My grandmother lived to be 100 years old and passed away at home surrounded by family and love. I cherish the time I was able to give back that same care my grandmother gave me and that's the mission of here at Heritage Oaks. Our mission here at Heritage Oaks is to unite families together and bring genuine care for our loved one's, in the comfort of their home. What we do at Heritage Oaks Structured Family Caregiving

Structured Family Caregiving (SFC) is a medicaid program that empowers you to care for your loved one in the comfort of their own home. This keeps your loved one safe and families together. This program offers weekly financial support dedicated to the care that you give.
