PPN vs TPN: quick vascular access reality check 👀

We hear this a lot:
👉 “It’s PPN, so a peripheral IV is fine.”

Sometimes.
But not always.

TPN is straightforward — high osmolarity = central access. No argument there.

PPN is where things get messy. Just because it can run peripherally doesn’t mean it should. If it’s running more than a few days, at higher rates, or on a patient who can’t keep an IV for more than 24–48 hours… that peripheral line is living on borrowed time.

This is why you’ll sometimes see a PICC placed even for PPN.
Not because we’re being aggressive — but because we’re being realistic.

Access decisions shouldn’t be based on labels.
They should be based on duration, osmolarity, vein quality, and patient safety.

Curious how your unit handles this.
Do you wait for IV failure, or plan ahead? 👇

Why do I love being a vascular access nurse? I encourage you to become an expert at something and then teach someone else. We can no longer gatekeep what we know.

As we look back at 2025 we pause and reflect on what made this year so special. Our people.

We want thank our nurses, providers, partners, and facilities for the trust you placed in us, the long nights, the teamwork, and the shared commitment to patient care. Every consult answered, every line placed, and every challenge worked through together added up to something bigger than any single moment.

This year brought growth, learning, and plenty of hard work, but it also brought connection, resilience, and pride in what we do. None of it happens without an incredible team behind the scenes and at the bedside.

We are grateful for each of you and excited to carry this momentum forward.

We closed out 2025 strong and we are ready for what 2026 has to bring.

Thank you for being part of our journey.
