Above and Beyond: Personal Touch Emergency Preparedness
Grace helped prepare her long time client Mr. Jones for the coming winter storm. In addition to making sure his generator was prepared as well as supplies in the home, Grace made some home-cooked meals. She knows his personal tastes so well, as he is very particular. She made sure he had several of his favorite meals to get through the weather. The caregivers from the home care were so grateful. Mr. Jones was able to weather the storm in the comfort of his home with his medical care taken care of as well as favorite meals.

Above and Beyond: Attending Body, Mind, and Spirit

Mrs. Jones was a "solo senior" who lived alone in her family home. Mrs. Jones had many medical conditions and doctor's appointments, which Emily helped with. Emily helped her eventually open up to the idea of moving in to an assisted living community. Mrs. Jones enjoyed the community much more than she expected. As she formed friendships, Mrs. Jones realized she had been substituting social connection with her doctor's appointments. Emily helped her reduce her anxiety and live a healthier and more fulfilling life. As Mrs. Jones approached the end of her life she was surrounded by her new found friends and Emily.

Above and Beyond: Saving Thanksgiving
Mr. Hines, had been experiencing cognitive decline leading up to the holiday season. He also had a history of getting urinary tract infections which increased his disorientation. As for many seniors, he would often end up the in the hospital.
The night before Thanksgiving eve, he became increasingly disoriented. His family was worried he would have to spend the holiday in the ER.
Andi, however, knew the contact information for a virtual ER doctor group who she works with closely often. The next morning, she was able to have mobile labs ordered along with a urinalysis. The virtual ER doctor was also proactively prescribed an antibiotic.
Mr. Hines' condition improved, so was able to avoid going to the hospital. And he was able to spend Thanksgiving with his family.

Above and Beyond: Work Place Disability Benefits Help

Jake had been receiving long term disability from his work following a stroke. Grace been been working with him during his recovery. Jake was distraught when he got a notice from his long term disability provider that it would be coming to an end. He appealed and provided all the requested records from providers, but it still was not enough. Grace composed a comprehensive Care Management report tying together all of his conditions, ongoing needs, and ADL and IADL functioning. With this thorough and comprehensive information, Grace was able to help Jake keep his much needed benefit.
