As we prepare for the upcoming school year, please note the following guidelines for requesting school medication forms:

Processing Time: Forms take 48–72 business hours to complete due to high request volume. Please plan ahead.

Eligibility: We can only process forms for patients seen in our office within the past 12 months.

Requesting Forms: When calling for a form, leave a detailed voicemail on our nurse line including the patient’s full name, date of birth, name of school, medication needed at school, and whether it will be kept in the nurse’s office or carried by the student.

Pick-Up: Forms must be picked up in person from our office.
