Heart failure is one of the leading causes of hospitalization for older adults and it rarely presents alone — most people with heart failure have at least one other significant cardiovascular condition. Without the right support, living independently becomes a challenge and many end up back in the hospital.

Our evidence-based Heart Failure Empowerment Program helps patients manage symptoms at home with a personalized care plan, interdisciplinary support and close coordination with physicians. Learn more: https://hubs.li/Q03QDJmH0

Meet Brittney, written by one of our nurses:
“Brittney is a kind, warm-hearted, and quick witted CNA who’s always ready to lend a hand. It’s easy to see how deeply she cares for her patients- always making sure they feel comfortable, respected, and truly cared for. The work CNAs do often goes unseen, but it’s absolutely essential. Brittney, along with our all our CNAs are such a crucial part of our team.”

Day 6 of celebrating National CNA week 6/12-6/18 and recognizing our CNA’s and their “why” hospice.
Meet Pam written by our nurse:
“Pam has the biggest heart and is always rooting for the underdog. She never judges and will go to the ends of the earth to make sure someone has what they need. I have worked with many people in my life but never anyone like Pam. Even when she is tired or not feeling good she treats each and every patient the way she would want to be treated. When we think heart of hospice Pan should come to mind. Pam does not do it for the recognition, she does it because she believes everyone deserves to be treated with dignity and love, no matter where they came from.”
Thank you Pam 💚

Day 3 of National CNA Week 6/22-6/18. We are celebrating our CNA’s and their “why” hospice:
Meet Kelley:
“I have been a CNA for many years and was excited to join the beacon team as I knew some of the folks that worked there. They really impressed me. Once I started being a hospice aide, I knew this was exactly what I was supposed to be doing. I especially love getting to know my patients and being able to spend time with them. Not just caring for them physically, but also emotionally. This work while it is very fun, is also very serious to me. I want my patients to have the end of life they wish for. I hope some day we can look at our loved ones and celebrate the end-of-life like we do newborns coming into the world. I have been blessed by some of the best experiences and some of these patients remain with me long after they’re gone. A little piece of them becomes a little piece of me. This job is the best job I’ve ever had and this team I work with makes me proud to be a part of them.”
Thank YOU Kelley for being a part of this team and for taking the time to bring peace at the end of life. 💚

In honor of CNA week 6/12-6/18 we would like to recognize our wonderful CNAs on our team and let them tell you their “why” hospice.
Meet Cheryl:
“I found passion in home care. It made me realize that it’s that one time that means so much. Listening to them tell you about their life, their family, their likes and dislikes can be quite interesting, but more importantly it helps me provide the care they deserve. Whether in a home or nursing facility everyone and every day is different. Whether it’s taking them outside, listening to old songs, painting their nails or holding their hand. It’s the one on one time that I love. And I have to say I love being a part of such a compassionate hospice team like beacon. And when I’m off I enjoy walking with my best friend Sadie.

Thank you Cheryl for being such a huge asset and always helping the team. You give care to our patients like they are your family. 💚
