Meet Our Team: This is Martie, our Patient Care Coordinator. Martie is one of our original employees and recently celebrated her tenth anniversary at SouthEast Hospice.

As our Patient Care Coordinator, Martie not only coordinates the intake process for all of our referrals, but also oversees our weekly Interdisciplinary Group meetings — a vital part to our individualized-care philosophy.

In her spare time, Martie enjoys spending time with her husband, four sons, and three grandchildren, and French Bulldog — Honey. You can often find her enjoying the beach during her time off.

Meet Our Team: This is Brooke, our Admissions Nurse. Brooke has eight years of hospice experience, with the past five being with SouthEast Hospice.

As our Admissions Nurse, Brooke is often the first contact our patients and their families have with our team. Her thoroughness and attention to detail during admission help the rest of our team provide the best individualized care possible.

Brooke loves assisting families fulfill a loved one’s final wishes.

In her free time, you can find Brooke reading a good book, spending time with her grandchildren, or enjoying a ride on her motorcycle.

Meet Our Team: This is Jordan, one of our Case Managers. Jordan has been with SouthEast Hospice for three years and has a true heart for hospice care.

As a Case Manager, Jordan is responsible for coordinating the care for her patients and their families. Like so many members of our staff, she has experienced the death of a loved one and understands the feelings and emotions her patients and families are facing.

One of Jordan’s strengths is her communication with patients, their families, and her colleagues.

Jordan is married and is quite the animal lover. Among her pets are five cats, three dogs, and her most recent addition — an iguana.

Meet Our Team: This is Sydneye, one of our Case Managers. Sydneye has five years of hospice experience, with the last two coming at SouthEast Hospice.

As a Case Manager, Sydneye coordinates and manages the personalized and compassionate care for her patients and their families. Her focus is on comfort, symptom management, and support during the end-of-life journey. She has an unwavering commitment to providing the best possible care.

Sydneye has a strong ability to display a calm and understanding manner during difficult times. Her weakness…Chick-fil-A.

In her spare time, Sydneye loves gardening and spending time with her husband and daughter at the beach.

Meet Our Team: This is Christie our Chaplain and Bereavement Coordinator. Christie has over 16 years of hospice experience, the past six with SouthEast Hospice.

Christie has the innate ability to meet each of our patients, and their loved ones, where they are in their spiritual journey. Christie becomes such a valued supporter to so many families, that she is often asked to participate in a patient’s end of life celebration. As our Bereavement Coordinator, Christie also provides ongoing support to families after a loved one has passed away.

One of Christie’s many strengths is her ability to bring a calming demeanor to each situation she encounters. Her weakness…her love of Candy Crush.

In her spare time, Christie enjoys spending time with her husband, two children, and three dogs. You can also usually find her with a good book in her hand.

Meet Our Team: This is Tina our Patient Care Manager. Tina has three years of hospice experience and prior to becoming our Patient Care Manager, she was one of our Case Managers.

In her current role, Tina oversees all aspects of patient care. She coordinates with Case Managers on patients’ medical needs, pain and symptom management, and communicating with patients and families.

Among Tina’s many strengths is her ability to face situations with a kind heart and loving manner. Her weakness…is everything Nothing Bundt Cakes has to offer.

In her spare time, Tina enjoys playing bocci ball and spending time with her husband and two children.
