Our residents had a blast playing Popcorn Pong! Aiming, tossing, and cheering each other on as they tried to land ping pong balls in popcorn tubs! Laughter and friendly competition made for a fun afternoon activity.

These smiles from our Halloween celebration were worth the wait!
What a spooktacular time we had at our Halloween party! Residents and staff dressed up for the occasion, enjoyed treats and laughter, and even had a surprise visit from “Elvis” himself for some rockin’ musical entertainment!
Thank you to everyone who joined in the fun and helped make this Halloween one to remember!

Today, we pause to honor the brave men and women who have served our country with courage, dedication, and sacrifice.
We are deeply grateful to the veterans within our Augusta Health and Rehab family — residents, staff, and loved ones alike — who have given so much in service to our nation.
SPC Boone, Personal Care Worker at Silverleaf Assisted Living, presented each veteran with a Certificate of Honor, and Ashley from St. Croix Hospice joined us to pin each veteran in a touching tribute of appreciation.
We are deeply grateful for the service and sacrifice of our veterans — today and every day. ❤️🤍💙

SilverLeaf Assisted Living residents and staff got into the fall spirit with a festive pumpkin painting afternoon! From bright and cheerful designs to spooky Halloween favorites, everyone showed off their creativity and had a blast together.
Moments like these remind us how much joy comes from sharing laughter, art, and a splash of paint with friends.

Danica is one of our lovely CNAs. So proud of her and grateful she is part of our team.

Congratulations to Danica Wolfe, daughter of Ashley and Lance Wolfe, on being selected as the recipient of the Good Citizen Award from the Daughters of the American Revolution (DAR)!

The DAR is a nonprofit, nonpolitical volunteer organization devoted to promoting patriotism, preserving American history, and securing our nation’s future through education.

Danica was chosen for this prestigious honor because she truly embodies the qualities of dependability, service, leadership, and patriotism in all that she does. Her teachers describe her as a student of exceptional character, with strong leadership skills, a dedicated work ethic, and a genuine commitment to her school and community.

We are incredibly proud of Danica for her achievements and the positive example she sets for others. Congratulations, Danica, and best wishes for continued success in all your future endeavors!

Residents and families came together for Operation Christmas Child!
Operation Christmas Child collects shoebox gifts filled with fun toys, school supplies, and hygiene items, then delivers them to children in need around the world as a way to share God’s love in a tangible way. For many of these children, the gift-filled shoebox is the very first present they will ever receive.
At Augusta Health and Rehabilitation, giving back is one of our favorite traditions. It’s even more special when it’s done together.

Our Story

Augusta Health and Rehabilitation is a senior living community well-known for skilled nursing care, assisted living, and rehabilitation services. Within a continuum of care, our thoughtful team supports those needing assistance with daily activities, medical needs, and end of life care. Residents have the security of knowing they can remain here without having to relocate and separate from their familiar surroundings and friendships, should their needs change over time.

Residents at Augusta Health and Rehabilitation have three delicious meals prepared for them each day. We also have a chapel on site for residents to utilize, as well as scheduled services for residents to attend to promote spiritual health. Additionally, Augusta Health and Rehabilitation has beautiful courtyards and outdoor spaces for our residents to enjoy.

We will happily provide the opportunity for you and your family to get to know our dedicated staff. Together, we’ll work on transitioning each new guest into one of our comfortable, new, state-of-the-art facilities.
