Maternity care is often disconnected from primary care, making it difficult for women to receive continuous care. This gap in care is concerning, especially for those who develop gestational diabetes mellitus, which can have long-term effects on the health of women.

Our team visited Wellstar Douglas, where we are collaborating across the state to understand why gaps in care happen and how we can improve the care for women with GDM and their children.
