Introducing Foundational Succession Planning for Families!

Foundational Succession Planning involves making a plan for the future of a family member with a disability. It often includes the process of passing over family leadership roles. For some families this may mean planning for another family member to take over caregiving responsibilities. For others, this may mean having to rely on more formal types of services and support options.

Generously funded by the Colorado Developmental Disabilities Council (CDDC), the Colorado Office of Employment First (COEF) is offering 50 parents or guardians, age 65 and up, individualized foundational succession planning.

This planning will take a person-centered approach that will focus on defining and connecting individuals, caregivers, and families with the essential and next steps when considering future care arrangements for their family member with an Intellectual and Developmental Disability (I/DD), once the caregivers and/or family are no longer able to be primary supports for the person.

The end result will start a plan for the future of a person with a disability.

Sometimes you have to play detective. Is your loved one too hot, too cold? Is it too noisy or quiet? Sundowning or in discomfort? What else could be causing erratic behavior? Or is it just the disease... "Together We Can Make a Difference." đź’ś
