VNA Health Care is a Joint Commission accredited Federally Qualified Health Center (FQHC) that is certified as a Primary Care Medical Home. VNA Health Care has fifteen (15) health centers serving patients in Kane, Cook, DuPage, and Will Counties. Services include: comprehensive primary healthcare; wellness and annual preventative services for men, women, and children; sick care, treatment of chronic diseases and acute illness; free pregnancy testing & complete pregnancy care, including obstetrical services; pediatric care; mental and behavioral health care; podiatric care; massage and physical therapy; pharmacy and 340-B Pharmacy Assistance Program; laboratory testing; dental services; and case management and care coordination. VNA also provides community wellness services including: WIC (A Special Supplemental Nutrition Program for Women, Infants & Children), Healthy Families America home visitation program & Doula services, and Better Birth Outcomes.
