TMF QIN Nursing Home Quality Improvement Let's share best practices and resources and collaborate with TMF QIN and other LTC providers.

Long-term care (LTC) providers are invited to join the TMF QIN Nursing Home Quality Improvement Facebook group to share best practices, collaborate with TMF QIN-QIO nursing home staff and other LTC providers, ask questions and problem-solve on topics associated with nursing home quality improvement. The TMF Quality Innovation Network Quality Improvement Organization, under contract with the Centers for Medicare & Medicaid Services, works with long-term care communities and other providers to improve nursing home quality and performance.

Start with easy changes like breaking up sitting time—set a phone or watch timer to stand and move every hour. Try leg lifts while watching TV. Parking farther away to boost your steps or taking a quick walk during phone calls can also help to make daily tasks count!

Remember to wear supportive shoes and check with your primary care provider before starting a new routine.

Every move matters so let’s keep moving!
