Burnt out PTs, this one’s for you.

I drove my 3rd grader to her field trip this morning. On a Wednesday . Not because I had a slow day. Because I built a cash-pay PT practice that actually works around my life.

I see fewer patients. I spend more time with each one. And I make more than I ever did grinding through a clinic schedule.

This is what cash-pay physical therapy can look like for the provider, not just the patient.

Comment MORE below and I’ll share a free cash pay resource from us directly to your inbox.

I was doing everything “right.”
Grad school. License. Clinic job.

And I was miserable.
Burnt out by Wednesday.
Counting down to the weekend.
Watching my PTO like it was a savings account I couldn’t touch.

I knew I didn’t go through 6 years of school to feel like this.
So I burned the playbook and built my own with Concierge Orthopedics.

Now I run a cash-pay PT practice on MY terms.
I take vacations without asking.
I treat patients I actually love working with.
And I wake up excited about my career again.

Your license is more powerful than they told you.
Send us a DM & we’ll see if you’d be a solid fit for our team!

🧩 𝐏𝐚𝐢𝐧 𝐡𝐚𝐬 𝐜𝐨𝐧𝐭𝐞𝐱𝐭—𝐧𝐨𝐭 𝐣𝐮𝐬𝐭 𝐜𝐚𝐮𝐬𝐞

Pain doesn’t always come from one clear injury or single source.

It shifts based on what your body and nervous system are dealing with in the moment.

At 𝐂𝐨𝐧𝐜𝐢𝐞𝐫𝐠𝐞 𝐎𝐫𝐭𝐡𝐨𝐩𝐞𝐝𝐢𝐜𝐬 𝐏𝐡𝐲𝐬𝐢𝐜𝐚𝐥 𝐓𝐡𝐞𝐫𝐚𝐩𝐲, care goes beyond the pain location—looking at movement, load, lifestyle, and how your body is responding as a whole system.

we need to talk about your foam roller.

if you’ve been grinding that thing up and down your outer thigh trying to “release” your ITB. I love the effort, but we gotta pivot.

here’s the thing: your ITB is not a muscle. it’s dense connective tissue, and research shows that foam rolling doesn’t actually change ITB stiffness at all. like, measurably zero change on ultrasound imaging.

why? because the ITB has no contractile fibers. it can’t “tighten” on its own and it can’t be “released.” what actually makes your ITB feel tight is the tension being pulled into it from the surrounding muscles and those are the ones that need the work.

rolling directly on the ITB also risks irritating the nerves and soft tissue underneath, so all that pain you’re pushing through? not productive. just pain.

roll here instead…
→ lateral quad (rotate slightly forward onto your front thigh)
→ lateral hamstring (rotate slightly backward onto the back side of your thigh)

foam rolling can absolutely be part of your routine. we’re not anti-roller. we’re pro-doing it right.

send this to someone who’s been torturing themselves on a foam roller and wondering why their IT band never feels better. they’ll thank you later

This is what concierge PT actually looks like.
No waiting rooms. No double-booking. No 8-minute treatment slots.
Just a fellowship-trained PT who picks up the phone, knows your name, and actually has time to fix the problem.
Cash-pay isn’t a luxury. It’s just what good care should feel like.

the Peterson step up was originally created in 1985 for World Cup skiers recovering from knee injuries. it’s been in clinical and athletic rehab ever since. We still use it because it works.

low bench is intentional. it trains terminal knee extension, the last few degrees of straightening your knee that you lose first after injury, surgery, or chronic pain. it also demands single-leg stability and control that most exercises don’t touch.

this is one of the first things I prescribe when someone walks in with knee pain.

save this. your knees will thank you later.

I know because I’ve been there.

Back to back patients. Notes at 9pm. Feeling like you’re doing everything right and still coming up empty at the end of the month.

That’s not a you problem. That’s a model problem.

Cash-pay physical therapy changed everything for me. Fewer patients. Deeper work. Better outcomes. And for the first time, a schedule I actually designed instead of one that was designed for me.

I put together a guide that breaks down exactly how the cash-pay model works and how to know if it’s the right move for your practice.
