ProsDent247: Year 4: 📷 The patient was super stoked to get his full arch implant fixed Zirconia restoration just in time before the end of the year holidays. It took us a year and some months of hard work including removing and replacing a failed implant just as we were ready the first time, and a lateral sinus augmentation to reach here where we were able to improve the patient's smile and ability to chew. Full arch Zr fixed implant restorations are a great option for people with failing dentition/ failing fixed teeth based restorations. Even though this is one of the best solutions available in modern digital dental arena, it is important to pay detailed attention to the hard and soft tissue parameters as well as biomechanical aspects to achieve long term and predictably successful outcomes. We can say this with great conviction: "These are not just for View, but fully healthy and functional to Chew".

ProsDent247: Year 4: It's not fun when your restorations turn up like this from your labs. Two different labs, same instructions provided, same poor crown design - one thing in common - End of the Year Rush. Using a custom healing abutment and/or custom interim restoration is a potential solution, but patients may choose not to have them due to finances or time needed, especially with posterior teeth. Designing custom abutments and crowns with Tissue expansion with a wide anatomic ideal emergence profile is an alternative and it works if the labs follow your instructions and are not rushing with cases or overlooking your instructions. This shows the importance of having a good lab tech to work with, and great lab communication.

ProsDent247: Year 4: Depth of Implant placement - continuation of the previous post: changed their mount design recently. The designs only differ slightly and still have markings on them to allow 1mm subcrestal placement as recommended by manufacturer. The new design has just one 360 degree uniform laser etched marking (vs two in the old design), to minimize confusion during surgical implant placement.

ProsDent247: Year 4: Day 1: Starting a new series. When you are working with a company like , it helps to have these implant mounts that have additional functions. During surgical placement, the markings assist with implant depth with respect to the edentulous crest. When the bony crest is level with the bottom marking, the implants will already be 1 mm subcrestal - HiOssen recommendation. Placing it to the next marking will place it 2 mm subcrestal. Thus making it predictable and convenient during surgery.

ProsDent247: Year 1: Day 72: Continuing the series. After the second set of provisional restorations were in use for 3 weeks & approved by the patient, we went ahead with the fabrication of the final restorations. The veneers on #8,9,10 and the implant crown on #7 looked good and the patient was very happy with the appearance. After minor adjustments, #7 Layered zirconia (screwmentable) and #8,9,10 E.max ( ) veneers were screwed in and adhesively bonded respectively (Rely.X Unicem 2 ). #12-14 had several issues so the temporary screw retained FPD was reinserted and the final FPD was sent back to the lab for remake.

ProsDent247: Year 1: Day 71: Continuation of the case from yesterday. After about 5 months of healing and implant uncovery, #8,9,10 were prepared for veneers (incisal overlap design) and temporized along with implant crown #7 and implant FPD #12-14. This is the second set of temporary restorations, after the first set had anterior provisionals that were really long inciso-cervically. After making multiple other adjustments, a second set of provisionals were made and patient allowed to use them for about 3 weeks. As you can see, the patient has an average smile line, and didn't really smile prior to us starting her treatment.
