Todo comienza con una cita inicial de Atención de Afirmación de Género para establecer tus metas y expectativas. En tus seguimientos trimestrales hablaremos sobre el manejo de medicamentos, así como los análisis y estudios de rutina.

¿Ya eres paciente? Habla con nosotros sobre cómo comenzar tu camino hacia la Atención de Afirmación de Género.

Si eres un nuevo paciente, una vez que se establezca tu atención médica, podremos iniciar tu proceso de HRT.

Nuestro programa de Manejo de Casos está aquí para ayudar a las personas que viven con VIH a encontrar y acceder a recursos que mejoren su calidad de vida en áreas como vivienda, acceso a alimentos, servicios dentales, apoyo para pagar servicios básicos, comprensión del sistema médico y mucho más.

Desde viajes en Lyft para citas médicas hasta asistencia financiera de emergencia, nuestros Administradores de Casos médicos y no médicos trabajan para identificar y cerrar las brechas que existen en este sistema roto.

¡Además, varios de nuestros administradores de casos hablan español e inglés!
