Dr. Fisher explains the key differences between placing implants under the muscle versus on top of the muscle.
Under the muscle (submuscular) gives extra implant coverage so edges and rippling are less visible and it often creates a more natural upper breast slope. 
On top of the muscle (subglandular) can give more projection and easier access with a typically quicker recovery, and it may be a good option for patients with enough natural breast tissue or those seeking larger immediate cleavage. 
Under the muscle may involve more muscle manipulation so recovery can be a bit longer and you might notice implant movement when the chest muscle contracts.

38-year-old patient with the BRCA1 gene came to Dr. Fisher after previous reconstruction attempts in another city were complicated by infection, leaving her flat following a preventative double mastectomy.
A thoughtful, three-stage reconstructive plan was created to safely restore her breasts:
1️⃣ Fat transfer to the chest wall to thicken and prepare the skin.
2️⃣ Tissue expander placement to gently re-stretch the skin envelope.
3️⃣ Final implant reconstruction with additional fat transfer for natural softness and contour.

Dr. Christine Fisher is a board-certified plastic surgeon specializing in breast reconstruction techniques for breast cancer patients using perforator flaps, such as DIEP flaps, and in cosmetic surgery of the breast and body. For appropriate candidates who are experiencing lymphedema after breast cancer treatment, she also offers lymphatic surgery, including lymphovenous anastomosis and lymph node transfer. She was named a 2017 Texas Super Doctor®.
Dr. Fisher is committed to providing the same care to her patients that she would want for her own family. Her goal is to provide personalized, compassionate care while safely achieving the best possible aesthetic outcome for each patient. Her artistry and keen attention to detail combine to make this possible. Dr. Fisher graduated medical school with honors from the University of Texas Medical Branch at Galveston and moved to Pittsburgh, Pennsylvania, to complete her training in plastic and reconstructive surgery. During her plastic surgery training, she cared for patients with breast cancer, head and neck cancer, and burn and extremity trauma, using both traditional and microsurgical reconstructive techniques to solve a spectrum of functional and aesthetic problems. She trained in microsurgical perforator flap (DIEP flap) breast reconstruction at Magee-Womens Hospital and studied microsurgical lymph node transfer to treat upper extremity lymphedema with Dr. Corrine Becker in Paris. Dr. Fisher has an affinity for the precision and artistry of plastic surgery and its potential to help improve and restore a patient’s form, function and confidence.
Dr. Fisher won the Gold Foundation Humanism and Excellence in Teaching Award twice and the Department of Plastic Surgery teaching award three times. She has published multiple peer-reviewed articles and book chapters in topics ranging from regenerative medicine to patient safety. Following her research fellowship in regenerative medicine, she continues to have an ongoing interest in the potential applications of adipose-derived stem cells and fat grafting in soft tissue restoration.
Dr. Fisher is proud to serve on the Medical Advisory Committee for the Breast Cancer Resource Center and is a regularly invited guest speaker on topics related to breast reconstruction, as well as an Art Bra® Austin sponsor. She regularly attends national breast and microsurgery meetings to stay up to date on the latest research and uses an evidence-based approach to provide optimal, state-of-the-art care.
When not in the hospital or clinic, Dr. Fisher enjoys spending time with her husband and three young children. She bakes on weekends, is energized by live music, and relaxes by jogging, swimming, going horseback riding, and participating in other outdoor activities.
