Dr. Menjivar and the Farmington Valley Orthodontics Team welcome all girl scouts to join us in an afternoon of fun and memorable experiences! Each troop will navigate through our office as they stop at fun, educational and interactive stations to learn about Dental & Orthodontic Health. Note our event will take one hour to rotate through four activity stations.
⭐All ATTENDING scouts will earn a Dental Health Certificate, create a cool dental craft, EARN A Dental Health Patch (these are considered fun patches not official Girl Scout patches) and take a group 📸 with Dr. Menjivar!⭐

Required information for Google Form: Troop #, number of scouts attending, troop level and troop leaders name and email address.
🔵SIGN-UP is on a first come basis
🔵A MINIMUM of 5 scouts required to secure a time slot! PLEASE check with your troop for a headcount prior to selecting a time!

🔵Troops that have signed up will be contacted for a final headcount and RSVP by April 18th.
If spots are filled, there will be a next up waiting list. Please comment below “Next Up Troop #" and provide the troop leaders email.
