🌿 Assisted Living Waiver (ALW) Program
Helping Your Clients Transition to Safe & Supportive Housing

Are you working with clients who need more support but want to avoid unnecessary nursing home placement? The Medi-Cal ALW Program offers a solution.

✅ Who Qualifies?

Adults with Medi-Cal coverage

Individuals who have spent at least 60 days in a nursing home OR can provide a letter from APS/Ombudsman

Must be able to transfer independently from bed to wheelchair

🏡 What’s Included?

Licensed assisted living placement

Daily personal care: bathing, dressing, grooming, meals

Medication management & health coordination

Social activities & community integration

24/7 caring staff support

📝 Case Managers: How You Can Help

Refer your client to an ALW-approved provider

Assist with paperwork:

Physician’s 602 form

Ombudsman support letter (if required)

CalAIM Program approval (for Anthem Blue Cross)
