This is something most people don’t realize until it happens to them.

Your body processes medications through enzymes (mainly in the liver), and those enzymes aren’t the same for everyone. Some people break medications down quickly, others more slowly.

That affects everything:
• how well the medication works
• how long it lasts
• whether you feel side effects
• and even if it feels like it’s “doing nothing”

So if you’ve ever thought: “Why did this work for them but not for me?” or “Why am I getting side effects and they’re not?”

You’re not crazy. It’s biology.

This is also why switching medications, adjusting timing, or even small dose changes can make a huge difference.

Allergy season is here and most people grab the first box they recognize. But antihistamines are not all built for the same job.

There are two main generations of antihistamines, and they work very differently.

First generation antihistamines like diphenhydramine (Benadryl) cross into the brain more easily. That’s why they work quickly but also cause sedation, brain fog, and that “I could fall asleep anywhere” feeling. They are helpful for sudden allergic reactions or nighttime relief, but they’re usually not ideal for daily daytime allergy control.

Second generation antihistamines like cetirizine (Zyrtec), fexofenadine (Allegra), and loratadine (Claritin) were designed to stay mostly outside the brain. That means they treat allergy symptoms while causing much less drowsiness and they last longer, often 24 hours.

The “big three” also behave a little differently:

Zyrtec (cetirizine): strongest symptom control for many people, but can cause mild drowsiness in some.
Allegra (fexofenadine): very balanced option with minimal sedation.
Claritin (loratadine): mildest and least sedating for many people.

The best choice depends on your symptoms, your schedule, and how your body reacts.

Today is World Obesity Day, and one thing we emphasize with patients is that weight management is about more than the number on the scale.

Understanding your body composition — things like body fat percentage and muscle mass — helps set the right targets and creates a healthier, more effective weight loss plan.

Rapid or “crash” weight loss can put stress on the body. Sustainable progress comes from consistent changes, proper monitoring, and preserving muscle while losing fat.

Medication can play a role for some patients, but education, guidance, and ongoing support are just as important.

Most energy products contain caffeine, but many also include other stimulants like synephrine, yohimbine, guarana, or high doses of B vitamins that amplify the effect. When you stack them, the total stimulant load adds up fast.

Here’s what’s actually happening physiologically:
Stimulants increase adrenaline and norepinephrine. That raises heart rate, constricts blood vessels, and can elevate blood pressure. For some people, especially if combined with stress, dehydration, or certain medications, that can trigger palpitations, anxiety, or sleep disruption.

What many people don’t realize:
• Caffeine has a half-life of about 5 to 7 hours. That 3 PM pre-workout can still be active at bedtime.
• Dehydration makes stimulant side effects worse.
• Some cold medications and ADHD medications already increase heart rate. Adding pre-workout on top can compound the effect.
• “Natural” does not mean gentler on your heart.

If you notice racing, chest fluttering, elevated blood pressure, or new anxiety after adding a supplement, that is data.

Before increasing your dose, switching brands, or adding another product, ask:
What is my total daily stimulant intake?

A crash is often about how FAST your blood sugar drops, not just the number itself. When you spike quickly and insulin rushes in, that rapid drop can trigger adrenaline and cortisol. That’s why it feels like:
• shakiness
• anxiety
• exhaustion
• irritability
• brain fog
• headaches

Your body reads that fast drop as a stress event. Even if your glucose is technically “normal.”

And here’s the part no one talks about:
It’s usually not just sugar.

Crashes are commonly linked to:
✔ high carb meals without protein or fat
✔ long gaps between meals
✔ caffeine on an empty stomach
✔ overcorrecting with quick carbs
✔ extreme restriction

Stability > restriction.

Pair carbs with protein and healthy fats. Eat consistently. Stay hydrated. And if crashes keep happening, it may be time to look at the bigger picture, including medications, hormones, or underlying metabolic patterns.
