If your child requires school forms to be filled out please drop them off at your earliest convenience or contact the office to see if they require an office visit. The typical turnaround time is 5-7 business days and we ask that you please pre-fill all patient/parent information beforehand.
A fee of $25 will be applied for the first signature page, and $5 thereafter for any additional forms.

To ensure our shot nurses know your time of arrival please ensure to write the time you sign in. Leaving the shot slip blank may cause confusion, and can potentially get skipped by not knowing which patient is next.
We want to ensure all patients get their shots in a timely matter, yet we take all of your considerations and feedback to better ensure our system. TIA

There was an incident this morning that caused the fire alarm to go off in the medical building. Medical personnel and patients were evacuated from the building to safety while firefighters on site determine the cause of the alarm.
