Thomas Nelson, DPM is a board certified podiatrist who specializes in diabetic foot care and podiatric surgery. He received his doctorate degree from the California College of Podiatric Medicine and completed his preceptorship with Robert Garner, DPM. Dr. Nelson has been in private practice for more than 30 years serving the Bakersfield area.

Byron Mui MD, MPH is board certified in Internal Medicine and subspecialized in Infectious Disease. He received his resident training at USC General Hospital and Infectious Disease fellowship at VA Wadsworth UCLA. Dr. Mui has been practicing both disciplines in Bakersfield for over 30 years.

Centennial Medical Group is now an approved COVID-19 vaccine provider with the California Department of Public Health! We received our first vaccine shipment this week. We received the Johnson and Johnson vaccine. One and done.

We have some great news to share with everyone. For several years, Dr. Yokoyama has been waiting for a kidney transplant and the day has finally come! To share the news, Dr. Y has made a video to update his patient community on his recovery.

Centennial Medical Group is excited to announce that it is now offering telehealth visits to new and established patients!

The Coronavirus (COVID-19) outbreak has created a serious disruption to all of our lives. Many patients are worried about leaving their home to travel to our clinic for a needed appointment.

In order to support our patients during this turbulent time, CMG is now scheduling appointments over live-video. If you are a patient that needs to be seen by one of our providers, but is concerned about leaving home, we can now schedule and treat you over a secure, HIPAA-compliant connection over Zoom.

CMG is pleased to welcome two new physicians to our medical group.

Mark Rivera, MD is board certified in Family Practice and has started seeing patients as of today, August 1st. Dr. Rivera will serve as a primary care physician to our adult and pediatric patients in English, Spanish, and Tagalog. He has been providing medical services to the Bakersfield community with Clinica Sierra Vista for the past three years.

Hootan Daneshmand, MD is a board certified plastic surgeon who specializes in cosmetic plastic surgery and breast reconstruction, including breast augmentation surgery. He has been in private practice for more than 20 years serving the Orange County and Kern County areas. Throughout his career, Dr. Daneshmand has been committed to traveling to poverty-stricken countries in order to perform surgical operations for the treatment of congenital defects in children.

Meet Our Providers: Carlos Alvarez

We are proud to announce Carlos Alvarez, MS, PA as the newest addition to the CMG family of providers. Carlos graduated with honors from CSUB with his BS in Biology in 2003, after which he completed his MS in Physician's Assistant Studies at Western University of Health Sciences in 2006. He is currently board certified with the National Commission on Certification of Physician Assistants.

Before CMG, Carlos worked at Clinica Sierra Vista where he provided medical care to local Bakersfield residents. Alongside his work at CMG, Carlos continues to work with Memorial Hospital where he provides urgent care for occupational medicine.

Janine Bourelle, MS, FNP-C is a Family Practice Nurse Practitioner and provides family-centered healthcare to patients of all ages. Her scope of practice encompasses health promotion, disease prevention, diagnoses and management of common and complex health care issues from childhood into adulthood.

She is proud to be a Bakersfield native, attending local schools through college and being able to serve the community in which she was raised. Janine graduated from California State University Bakersfield with her Bachelor’s in Science of Nursing in 1997 and worked as an RN for local hospitals, home health, and The Centre for Neuro Skills working with people with brain injuries. During her time at CNS she served as the director of Nursing. She earned her Master’s in Science of Nursing in the Nurse Practitioner program at California State University in 2001.

After becoming a Family Nurse Practitioner in 2001, she began working at Centennial Medical Group. Janine continued to work at the CNS as a Nurse Practitioner providing health care for clients for acute illnesses as well as health maintenance physicals until 2003. She has since worked full time for Centennial Medical Group.

Janine has the unique combination of strong clinical skills and interpersonal skills, taking the time to listen to her patients to acquire the information needed to provide the best possible care. She strives to build long-lasting, trusting relationships with her patients allowing her to understand their particular needs and provide a comprehensive approach to managing their healthcare.

Meet Our Providers: Nelson Madrilejo, M.D.

Nelson Madrilejo, M.D. is an Internal Medicine physician who sub-specializes in Endocrinology.

Dr. Madrilejo was born and raised in Lima, Ohio. He graduated from University of Notre Dame with a B.S. in Pre-Professional Studies in 1981. After graduating from University of the East Medical School in 1986, he completed an Internal Medicine residency at St. Barnabas Hospital in Bronx, New York. He then went on to complete a Diabetes & Endocrinology fellowship at the prestigious University of Southern California School of Medicine.

As a private practice physician with over two decades of experience in internal medicine and endocrinology, Dr. Madrilejo treats adults and young adolescents with endocrine disorders, thyroid diseases, and diabetes-related issues.

Meet Our Providers: Larry Yokoyama, M.D.

Larry Yokoyama, M.D. is a Family Practice doctor who was born in Ann Arbor, Michigan, but grew up in the Aloha state of Hawaii. He earned his bachelor's degree from the University of Hawaii and went on to receive his medical degree from University of Hawaii, John A. Burns School of Medicine.

After a brief period of private practice in Richmond Hill, Georgia and Kailua, Hawaii, Dr. Yokoyama settled in Bakersfield, California in 1991. As co-founder of Centennial Medical Group, he provides care to adult and pediatric patients in his downtown Bakersfield clinic. In addition to being a Family Practice physician, Dr. Yokoyama held numerous positions throughout his career as an Emergency Room physician.

Dr. Yokoyama also holds multiple preceptor programs to receive Physician Assistant students, and to provide future healthcare professionals with up-to-date medical knowledge in their clinical training.
