β¨ Now Hiring: Wound Care, Nurse Coordinator (WCC-LVN) β¨
π Position Description:
The Wound Care Nurse Coordinator (WCC) provides and organizes nursing services, implements the agencyβs ostomy and wound clinical specialty program, and ensures quality care for Home Health patients. The WCC evaluates, manages, and educates on wound and ostomy care while coordinating care with physicians, caregivers, and the interdisciplinary team.
π©Ί Key Responsibilities:
β’ π₯ Receive and follow up on referrals per agency protocols
β’ π©ββοΈ Monitor patients at risk for pressure wounds; evaluate new and declining wounds
β’ π Complete assessments and update care plans
β’ π Provide wound/ostomy education to patients, caregivers, and staff
β’ π Develop and update wound care protocols and patient education materials
β’ π¬ Document and communicate patient care needs to physicians and supervisors
β’ π Participate in quality improvement initiatives and case conferences
β’ π€ Provide clinical guidance to RNs, LVNs, and CHHAs

Ensure compliance with HIPAA, CMS wound guidelines, and agency standards
π Minimum Qualifications:
β’ π©Ή Knowledge of wound care, ostomy care, and home health protocols
β’ π€² Strong interpersonal skills; ability to work with diverse patients and staff
β’ π Ability to maintain confidentiality, manage stress, and problem-solve independently
β’ π» Competence in documentation, computer use, and wound measurement
π Experience and Education:
β’ π₯ Current LVN license with WCC/WCON or equivalent certification in California OR willingness to obtain during employment
β’ β±οΈ One to two years of acute med/surg, Home Health, or wound clinic experience
β’ β€οΈ Current First Aid and CPR certification
β’ π Valid California driverβs license, reliable vehicle, and insurance
