Autumn Lake at Long Green is located in Northern Baltimore in the residential community of Stoneleigh and Lake Evansham. Easily accessible by public transit and surrounded by greenery, the facility is situated on 5 acres of wooded property with numerous outdoor courtyards, lounges, and patios. In addition to sub-acute rehabilitation, we are known for our excellence in long-term care, providing round-the-clock skilled nursing for a wide range of complex medical conditions.
