All correspondence should be addressed to PO Box 36, Riva , MD 21140. Voice Messages can be left on Google voice (Telephone number 410-871-8231). To expedite a records request, a photocopy of your signed request can be texted to the google voice number. Include your date of birth , address and phone number. Please also provide a fax phone number to fax records to. Small records ( less than 5 pages) can be texted to your smart phone if you also release the physician from HIPPA privacy policy, as this is not a secure electronic mode to send records. Records can be faxed without charge if less than 30 pages.Payment for all mailed records must be recieved prior to copy and mailing. IF you leave a recorded message from a smart phone on the google voice line we can text information to your phone, including the number of pages in your record and the charges to mail them to you. Please be advised that records are shredded 7-10 years after your last encounter with the physician.
