With the start of the new year often comes changes to insurance plans—this may include new member ID numbers or entirely new policies. To help prevent billing delays or errors, we kindly ask that you notify our office of any insurance changes as soon as possible.
If insurance on file is inactive and updated information is not provided at the time of your visit, your appointment will be processed as self-pay, and payment will be required at check-in.
