We are a 102 bed skilled nursing facility offering a full continuum of care including MeadowPark Recovery Care, MeadowWood Long Term Care and MeadowLane Assisted Living. MeadowPark is St. Clare Meadows Care Center’s Recovery Care Unit, offering new pati

ents a more private setting when recovering from a surgery or illness. Patients that choose MeadowPark for their recovery experience person-centered care where staff understand and respect each person’s individual needs and preferences. Our highly trained nursing and rehab staff work with you to develop short and long term goals focused on returning residents to their greatest level of independence possible. MeadowPark offers 22 private and 8 semi-private rooms, each with a private bathroom and shower. Rooms are located near the Rehab Department gym which offers state of the art therapy services including physical therapy, occupational therapy and speech therapy. Our interdisciplinary team works together to provide individualized care for each patient and will assist in arranging preparations for discharge. MeadowPark residents enjoy their own dining room which offers at-will dining where patients can choose when and what to eat. Vaulted ceilings and large windows allow for natural light to fill the space. A separate activity space allows patients a place to watch the game on the big screen TV, put together a puzzle, read the newspaper, and even cozy up to a warm fire next to the fireplace. Special activities are offered, including happy hour and special meal (provided by local restaurants), just for MeadowPark residents. MeadowWood Skilled Nursing:
MeadowWood is St. Clare Meadows Care Center’s long term care unit offering private and semi-private rooms. Resident rooms are all centrally located to the nurses’ station, dining room, activity lounge and chapel. Residents that call MeadowWood home experience person-centered care where staff understand and respect each person’s individual needs and preferences. Residents on MeadowWood enjoy at-will dining, which allows for them to choose where and who they would like to sit with and what time they eat, as well as offering a wide variety of food choices fresh from the buffet. MeadowLane:
MeadowLane is St. Clare Meadows Care Center’s assisted living unit. Residents and families choose MeadowLane as their home for many reasons…. the companionship it kindles, the organization it gives to one’s day, the assistance it provides for independent living. But most importantly, individuals choose MeadowLane for the peace of mind it provides to families knowing that someone they love is receiving exceptional care in a trusted, supportive and homelike environment. Residents that call MeadowLane home experience person-centered care where staff understand and respect each person’s individual needs and preferences. MeadowLane offers private and semi-private rooms for those residents that need assistance to live independently. Most rooms offer a kitchenette including a microwave and refrigerator. Every room features a large living area, storage closet and private bathroom complete with a walk-in shower. Residents are encouraged to furnish their rooms with personal items, mementos and favorite furniture. A twin bed, nightstand and dresser are provided, if desired. MeadowLane residents have their own dining room with family style dining. They are also welcome to participate in activities and chapel services throughout the entire St. Additional activities are provided weekly in the MeadowLane common area just for MeadowLane residents. Residents are invited to participate in a wide variety of daily activities including exercise classes, music and singalongs, pet visits, intergenerational activities, baking/cooking, reminisce, bowling, games, crafts, church services and bible studies, travelogues, van outings, current events, happy hour, seasonal events and more. Facility pets add to the homelike environment. An outdoor patio offers residents a place to plant flowers and small vegetables in the warmer months.
