If you are on the patient portal you can now upload pictures of your insurance cards, drivers license, medical documents and images safely and directly to your chart! This is much safer than sending information in email.

We have the 2024 Holiday Schedule done extra early so hopefully things will operate more smoothly this year!

With the Holiday break coming up we wanted to remind you refill requests can be left on voice-mail (304-521-1914 ext 402 or 403) or through the portal. Please only leave one message! We will get to them as quickly as we can and multiple calls for the same thing just take up valuable time. If leaving a voice mail please leave your name, date of birth and what you need, if we need more information we will call you. Please don't leave messages saying "This is Jane Doe, please call me back." Leave what you need in the message. If you are having an emergency please call 911 or go to your nearest emergency department. We hope you all have a Happy and safe Holiday Season!!!
