Support from dementia-trained nurses for ongoing guidance and check-ins

24/7 access to a dementia-trained professional for questions or concerns

Help coordinating care with doctors, specialists, and medical suppliers

Medication review for safety and proper management

Education, coaching, and emotional support for family caregivers

Individuals who:
• Have a dementia diagnosis
• Are enrolled in Medicare Parts A & B
• Live at home or in a senior living setting (independent, assisted living, or memory care)
