As another reminder, we are required by all medicaid programs to have a copy of your insurance card on file.
If you come to your appointment without your insurance card (Wellcare, Humana, Aetna, Passport, UHC) we will not be able to see you.
You must provide valid ID and eligibility cards per our contract with medicaid.
A "Team Kentucky" card does not provide the information we need to verify eligibility and submit claims.

Team Kentucky cards are only valid for members with Traditional FFS Medicaid, which means you do not have Wellcare, Humana, Aetna, Passport, or UHC)
