RAPID COVID TEST CAN BE MISLEADING!

Rapid test is as good as your thermometer. It is only useful when you need a negative test to travel or to go back to work or school (we do not recommend this). Right now if you have ANY of the following symptoms: runny nose, nasal congestion, cough, headache, fever, diarrhea vomiting etc. then you may safely assume that you have COVID infection.

We have COVID PCR tests for our patients. We now are able to get the results back in 24 hours.

We still offer the rapid COVID tests if patients request them. We always inform our patients about the poor quality of such tests.

Now is the time to choose whether to send your children to school or not. It is hard to make a decision. There are pros and cons on either side. However, please do not send your children to school if any one in your family having the following medical conditions which may make them at high risk for severe COVID-19.

Cancer
Chronic kidney disease
COPD (chronic obstructive pulmonary disease)
Immunocompromised state (weakened immune system) from solid organ transplant
Obesity (body mass index [BMI] of 30 or higher)
Serious heart conditions, such as heart failure, coronary artery disease, or cardiomyopathies
Sickle cell disease
Type 2 diabetes mellitus
Asthma (moderate-to-severe)
Cerebrovascular disease (affects blood vessels and blood supply to the brain)
Cystic fibrosis
Hypertension or high blood pressure
Immunocompromised state (weakened immune system) from blood or bone marrow transplant, immune deficiencies, HIV, use of corticosteroids, or use of other immune weakening medicines
Neurologic conditions, such as dementia
Liver disease
Pulmonary fibrosis (having damaged or scarred lung tissues)
Smoking
Thalassemia (a type of blood disorder)
Type 1 diabetes mellitus

Dear Governors,

We are a pediatric office in Hillsborough, New Jersey. We are deeply concerned with the current Coronavirus situation in the United States. We worry that the number of Coronavirus deaths will pass 1 million in just a matter of time if we do not take action now. Our economy will tank as a result of the pandemic. We only have limited time to act on it because Coronavirus transmission has been shown to occur exponentially. All states have to mount a coordinated team effort to tackle this problem as positive cases of Coronavirus can move freely between states or from other countries. In many parts of the US where certain states have reopened their businesses, an alarming number of people do not take precautions at all. We see on TV and through other news media that many people are in crowds without facial masks. It came as no surprise that the number of new Coronavirus cases skyrocketed in those places. Here are my suggestions to all of you:

1. Implement a massive campaign to educate the general public. People need to know that Coronavirus does not behave like flu infection. It doesn’t go away in the summertime. It can cause severe illnesses and extremely higher death rate than regular flu infection. Young people can die from it too. People need to be educated that opening up the economy does not mean that life goes back to normal. People still need to wear facial masks in public, keep social distancing, and vigorously wash their hands. They should avoid unnecessary trips to public places. Partial lock downs are essential if the Coronavirus cases suddenly surge after opening of business.

2. We need to advocate for self-quarantine for people traveling to different parts of the country, especially people from coronavirus hotspots to travel to other states. We must implement a mandatory quarantine for international travelers from active coronavirus areas. We also need to carefully check each international travelers’ travel history in the past 2 weeks to see if they have traveled to hotspots. We need to close borders with countries like Brazil. Finally, we should also ban travelers from entering the US, who had been to Brazil in the past 2 weeks and come to the US via a third country.

3. More testing sites need to be set up. This should be followed by vigorous contact tracing, and self-quarantine. Any opening of business without adequate testing, meticulous contact tracing and mandated self-quarantine is an invitation for disaster. We can recruit volunteers for each township and each neighborhood in order to fulfill this effort.

4. Governors need to coordinate the production of more PPE‘s in the states. Ideally this should be coordinated at the federal level. However we have not seen that happening. As we have more Coronavirus cases, the need for N95 respirators, facial masks, gloves, isolation gowns, alcohol solution and etc. will surely increase. It is disappointing that doctors still cannot purchase N95 masks. Alcohol solution is still a precious commodity. We are now at war practically. Therefore, it is justifiable to mobilize resources in your states to convert businesses to produce PPEs. They are not that difficult to manufacture if our governors take good leadership.



5. Governors need to communicate with the hospital directors regarding the capacity of hospital beds and the ICU beds availability. Hospital directors should work to increase their capacity to handle the influx of COVID-19 patients so that our healthcare system will not collapse.

6. We might need to create community centers that are designed for people who have been tested positive for COVID-19 and live in congested environments where self-quarantining is not possible. We may also use these centers to manage selective COVID19 patients to ease the pressure on the hospitals.

We don’t have much time left. Fall and winter seasons will be here in a few months. By then, there will be regular influenza viruses and seasonal common cold viruses to complicate the whole picture. These viruses will send more patients to the hospitals which may have already been congested by the Coronavirus patients. Furthermore, these regular viruses will weaken the patients’ immune system and make them more susceptible to coronavirus infection.

The weather will be great tomorrow. We will have an outdoor office set up in the courtyard of our office complex. We will see patients who need vaccines and newborn check ups. The exam table will be disinfected before and after each patient. Everyone will be required to wear a mask. There shall be ZERO transmission of Coronavirus. Novel coronavirus disease (Covid-19) is now a worldwide pandemic. This virus causes more deaths than the flu infection (10-20 times more lethal) and is unfortunately much more contagious than the flu.
Main symptoms of Covid-19:
Severe cough (dry cough or productive cough), fever and shortness of breath. Some patients may have runny nose, throat pain or diarrhea.

Severity of Covid-19:

20% of patients who are sick will need hospitalization, including admission to ICU. About 1% of sick patients may die from Covid-19 related respiratory failure or multi-organ failure. Death rate is more than 8% in Italy as of 03/20/2020. Death rate is much higher when there is influx of too many patients to the health care system at the same time.

Who are at risk:

Everyone! Not just the old population or people with diabetes, hypertension or other underlying medical problems.

More patients in the ICU for Covid-19 infection are younger than 60 years old in France.

Many previously healthy young doctors and nurses have died from Covid-19 in China.

Covid-19 may cause permanent lung damage.

Patients with asthma:

Patients with asthma are at high risk for severe Covid-19 infection. It is especially important that such patients go over their asthma action plan with their primary care doctors. Their doctors shall determine whether they need to use preventive medicine, such as inhaled corticosteroids. Besides taking precautions to prevent Covid-19 infection, asthma patients should avoid triggers to their asthma.

Asthma patients should inform the hospital doctors about their medical history. This will help the doctors optimize their treatment plan, such as inclusion of systemic steroids in their treatment protocols.

Sometimes, asthma can be difficult to diagnose. Patients may simply present with prolonged stubborn cough after a respiratory illness, or severe cough after exercise. Some patients present with cough after being outdoors when it is windy or with high pollen counts.

Dr. Jiang is very experienced in asthma management. If you have asthma or have asthma like symptoms, you may request a consultation with Dr. Jiang today so you will be well prepared in this Covid-19 pandemic.

Prevention of Covid-19:

Coronavirus is very contagious. Patients may spread the disease even during incubation period. Social distancing is important. Avoid public places and keep 6 feet of distance from each other.

Wash your hands often and thoroughly. Try not to touch your face in public places.

Covid-19 is a severe respiratory illness. Therefore wearing facial masks in public places is important. Please reserve N95 masks to frontline health care workers.

Eat a balanced diet. Eat well cooked food. Covid-19 patients shed viruses in their stool, potentially causing f***l oral transmission of the disease.

If you are not feeling well, call your primary care doctor. Do not go to the ER unless you are experiencing a medical emergency. Hospitals may get overcrowded during Covid-19 pandemic. Patients may get cross contamination with coronavirus.



It is very important that all of us take precautions so we do not go to the hosptals at the same time. We should avoid running out of hospital beds, ventilators, and protective gear for health care workers.
