Reach is an established home health agency.

Our organization was developed in response to patients who needed assistance with health care and personal needs and wished to remain in an independent living environment. We provide a cost-effective alternative to skilled care by utilizing highly trained caregivers as an integral part of our healthcare delivery system. Starting with a thorough evaluation by a Registered Nurse (RN) Case Manager, w

e determine the patient’s health status and medical/personal care needs. The RN will determine if the patient’s needs can be met by an unlicensed caregiver based on the stability of the patient’s status, the nature of the tasks, and the training of the caregiver. The RN will develop a plan of care for the patient including the frequency of supervision required.

Our Story

Reach Healthcare Services is a Licensed and Certified Home Healthcare Agency specializing in care for the Geriatric patient. Our clinicians and staff enable patients in Harris and the surrounding counties to live independently and/or have skilled medical care in their homes and places of residence. As a Registered Nurse owned and managed company, the expert professional nursing care offered by Reach is a central feature of the company's founding and mission.

The company was initially founded in 1987 to provide Skilled Private Duty Nursing services in the home setting. These skilled services were primarily for patients dependent on ventilators or having care needs for tracheostomies, G-tubes and infusions that were a result of progressing diagnoses, catastrophic injuries and other co-morbidities. The company's services were expanded shortly thereafter to include Nursing and Therapy Visits for the Medicare population. To complete our Home Healthcare continuum, we developed our Non-Medical Caregiver Services Program.

Our passion at Reach Healthcare Services is to support our patients’ choice to safely stay in their home by providing the appropriate level of care to meet their changing healthcare needs. As we work to meet our patients’ needs, we strive daily to consistently demonstrate our core values.
